❖Have you been in contact with a confirmed or probable Covid-19 case in the last 14 days?
❖Have you experienced any respiratory symptoms recently? (fever, shortness of breath, cough, sore throat, loss of smell, runny nose, sneezing)
❖Does anyone in your bubble feel unwell with respiratory symptoms?
If the answer to any of these questions is yes we are unable to see you at the clinic and will need to see you via telehealth instead.
Please make an appointment with your GP if you are unwell or phone Healthline 0800 358 5453.
Please inform us before your appointment if the answer to any of these questions changes.
❖Are you immune-compromised in anyway?
If the answer to this question is yes, then please consider a telehealth online appointment. If you really want a face-to-face consult, please contact us so that we can discuss this with you.
